Hi, I’m Jae Antonio (they/them).

My background is in Mental Health, Business Administration and Clinical Social Work. I was born and raised on the island of Oahu and have lived various places on the west coast including Portland Oregon and Fort Collins Colorado, finally replanting my roots in beautiful Northern California.

I have spent the last 20+ years serving individuals with disabilities of all ages navigate through school, work, relationships and other life transitions to meet their personal and professional goals. As an Associate Clinical Social Worker (ASW), my career has focused on Specialized Mental Health and serving Transitional Age Youth (TAY) & the Disability and LGBTQIA+ communities. I have completed intensive training to provide crisis response, therapy, case coordination and trauma-informed care.

In my roles as an independent facilitator, advocate, therapist, coordinator and behavioral coach, I use collaborative and integrative skills and a holistic approach. I strongly believe that everyone should be viewed from a strengths-based person-centered lens, and that the Self-Determination Program (SDP) truly allows a person to “think outside of the box” and access services and resources that fit their unique skills, needs and talents. I am extremely eager to help those interested in SDP navigate through their journey and discover all the program has to offer in ultimately creating the life they want and deserve.

Hi, I’m Coleen Antonio (they/them).

I am a former special education teacher with 10 years of experience supporting learners in primary, secondary and adult living skills and over 30 years living and working in the disability community in many different roles and from many different perspectives. As a career services provider, my approach is influenced by Sunny Hansen’s Integrative Life Planning theory; looking at career and life planning in a holistic way.

My graduate work focused on increasing competitive integrative employment opportunities for individuals with significant support needs and the need for IEP transition planning to begin earlier and in a more structured, integrated way through middle school and high school, ensuring proper supports are in place for transitions after high school and beyond.

As a family member of an SDP participant, I have working knowledge of the program; its promises and pitfalls, legislative changes that affect the program, how to navigate the set-up and ongoing administration of the program, and the relevant stakeholders who can provide support when needed. I look forward to helping people navigate the program to enrich their lives and reach their goals.
